Render fillable pdf form in angularjs spring mvc application

I would like to show fillable pdf form (example an I-9 form) which user will fill. On Submit, I'd like to extract the user-filled data. I am looking at pdf.co for help with this.

My application is based on AngularJS + Spring MVC.
